Professional Needs Assessor - Community Care Service

Join a dynamic and supportive team to deliver comprehensive assessment and support planning for older people in the community.

Job Description:

We are seeking a skilled and empathetic Professional Needs Assessor to join our multidisciplinary team. As a key member of our community care service, you will conduct thorough assessments of older individuals' medical, physical, cognitive, psychosocial, cultural, environmental, and social needs.

Using the Integrated Assessment Tool (IAT), you will develop tailored support plans that empower clients to live with dignity and independence.

Key Responsibilities:
  • Conduct home-based assessments for clients referred through various channels
  • Formulate support plans in collaboration with clients, carers, and relevant services
  • Accurately document findings and recommendations
  • Work collaboratively as part of a multidisciplinary team to ensure seamless client care
Required Skills and Qualifications:

To succeed in this role, you will require:

  • Experience working with older adults in a community or health setting
  • Strong time management skills with the ability to prioritize effectively
  • Excellent written and verbal communication skills
  • The ability to work independently and collaboratively

Additional Information:
