Experienced Level 2 Support Coordinator
Job Description:We are seeking a highly skilled and experienced level 2 support coordinator to join our team. Our ideal candidate will have a strong background in support coordination, with excellent communication skills and the ability to work effectively with participants, families and other stakeholders.
The successful applicant will be responsible for providing high-quality support to participants, including developing and implementing individualised support plans, coordinating services and supports, and building positive relationships with participants and their families.
Our team is committed to delivering exceptional outcomes for participants and we are looking for someone who shares our vision and values.
Required Skills and Qualifications:- 1 year of experience in support coordination or LAC;
- NDIS workers screening (preferred);
- Driver Licence (required);
- Ability to obtain prior to commencing: NDIS Screening, WWC, A reliable car with suitable car insurance; and
- Intermediate computer skills.
