Community Health Specialist

We are seeking a passionate and motivated Physiotherapist to join our dynamic community-based team on the Sunshine Coast. This role will enable you to work to the top of your clinical scope, providing quality care across a variety of work settings and caseloads.

The ideal candidate will have excellent communication skills and be able to deliver evidence-based assessments, recommendations and active interventions for a range of programs in the comfort of our customers' homes.

Key Responsibilities:
  • Home-Based Rehabilitation: Deliver acute and sub-acute rehabilitation services in the home through hospital substitution programs such as Rehab at Home and Hospital in the Home.
  • Aged Care Programs: Support our ageing population to stay well and independent at home through Government Aged Care programs such as Home Care Package, Short Term Restorative Care, and Commonwealth Home Support Programs.
  • Collaboration and Supervision: Work independently in the community and collaborate closely with our multi-disciplinary team for optimal patient outcomes, and engage in our clinical supervision program with a designated supervisor.

Requirements:

  • Preferred Experience: Community or hospital Physiotherapy experience.
  • Qualifications: Physiotherapy degree with unrestricted registration.
  • Valid Driver's Licence: Valid driver's licence and insured roadworthy vehicle required.
