Role Overview

This Senior Practitioner position plays a pivotal role in our team, overseeing the assessment and management of clients referred to the Social Health Commonwealth Psychosocial Support Program (CPSP) with complex mental health needs and/or high risk.

The successful candidate will provide leadership and support in clinical supervision, working closely with our CPSP, Social Health Care Coordinators, and Home and Healthy Program teams.

Key Responsibilities
  • Caseload Management: Effectively assess and manage a caseload of clients, providing quality support and ensuring timely interventions.
  • Data Management: Maintain accurate data records, utilising LogiQC quality management systems to enhance service delivery and performance monitoring.
  • Leadership and Supervision: Provide guidance and support to team members, promoting professional growth and development within our multidisciplinary team.
  • Client Engagement: Foster positive relationships with clients, facilitating access to holistic care through coordination with internal and external services.
  • Communication and Collaboration: Work effectively with colleagues, community organisations, and government agencies to ensure seamless referrals, networking, and information sharing.
Requirements
  • Tertiary qualifications in allied health (e.g. social work, psychology or mental health occupational therapy).
  • Minimum 5 years' experience in providing support and case coordination to clients with complex mental health presentations.
  • Demonstrated ability to apply high-level practice judgement when working with clients in setting and achieving goals.
  • Ability to work with culturally and socially diverse groups, applying a culturally responsive framework to guide interventions.
  • A willingness to travel across the ATSICHS Brisbane region and work across different sites.
Preferred Qualifications
  • Current, full registration with the appropriate professional body (e.g. AHPRA, AASW).
  • Experience in delivering supervision or leadership activities.
  • Previous experience in a multi-disciplinary team environment.
Compensation Package

We offer an attractive compensation package, including a base salary of $97,724 per annum, plus 11.5% Super and access to salary packaging opportunities.
