We are seeking a dedicated and skilled Occupational Therapist to join our Memory Support Transition Service Team.
The Occupational Therapist will participate as a member of the Memory Support Transition Service team, providing support to patients with memory loss and Behavioural and Psychological Symptom of Dementia (BPSD) with the aim of safely transitioning them from the Acute and Subacute inpatient environment into community accommodation.
This role involves working closely with the Geriatric Evaluation and Management (GEM) Nurse Practitioner Candidate and the Multi-D team to develop and action patient-centred management plans to support patient well-being and transition to new environments.
About You- A degree or other qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia
- Experience as a clinician in the assessment and provision of high-quality Occupational Therapy services to clients with a variety of conditions requiring management of issues associated with cognitive impairment and/or changed behaviours
- Proven experience in delivering high-quality and safe care consistent with the National Safety and Quality Health Care Service Standards
- High level interpersonal, negotiating, counselling, and problem-solving skills with the ability to communicate with patients, other professional staff, and community agencies
- Provide liaison to Residential Care Facilities to support the integration of care between SA Health and the community setting by providing direct patient care and educating and supporting RCF staff to care for patients with memory loss and responsive behaviours in line with the patient's documented behaviour support plan
- Work closely with the GEM Nurse Practitioner Candidate and the Multi-D team to develop and action patient-centred management plans to support patient well-being and transition to new environments
- A competitive salary range of $88,255.00 - $102,240.00 per annum, pro rata
