About the Role

This position will provide quality client-centred support and case management to young people aged 15-18 years old who are in care. You will work closely with clients to establish effective relationships and ensure they receive the support they need.

Key Responsibilities:

  • Deliver Support Services: Provide support to young people in line with Youth Services Model of care and associated Frameworks.
  • Establish Relationships: Establish effective relationships with clients that encourage self-determination and participation.
  • Provide Case Management: Ensure case management support is provided to clients in accordance with TSA case management models and standards of practice.
  • Maintain Client Records: Maintain accurate and up-to-date client records using TSA information systems.
  • Support Access: Assist clients to access support services and establish community supports as needed.

Requirements:

  • Certification: Minimum Certificate III or IV in Youth Work/Human Services.
  • Experience: Relevant equivalent experience in the Human Services Industry.
  • Additional Requirements: Experience working with youth in a community or residential setting desirable.
  • Working With Children Check: WA Working with Children Check required.
