Chronic Disease Management Nurse

We are seeking a skilled Chronic Disease Management Nurse to join our team at ForHealth Group (Dayforce). As a CDM Nurse, you will play a vital role in supporting our patients with chronic conditions, empowering them to take ownership of their health and well-being.
Key Responsibilities- Develop and Implement Care Plans: Collaborate with our healthcare team to create personalized care plans for patients with chronic conditions, ensuring they receive the necessary support and resources to manage their health.
- Health Assessments and Coaching: Conduct thorough health assessments to identify potential health risks and provide coaching to patients on healthy behaviors and lifestyle changes.
- Team Collaboration: Work closely with our multidisciplinary team, including GPs, to ensure seamless care coordination and promote preventative healthcare practices.
- Administrative Tasks: Manage administrative duties related to CDM appointments, including data entry and record-keeping.
- AHPRA Registration: Must be registered with the Australian Health Practitioner Regulation Agency (AHPRA) as an Enrolled or Registered Nurse.
- Experience in CDM: Proven experience in Chronic Disease Management, including health assessments and care planning.
- Strong Communication Skills: Excellent communication and interpersonal skills, with the ability to build strong relationships with patients and healthcare professionals.
