About the Role

We are seeking a highly skilled CDM Nurse to join our team at ForHealth Medical Centres. As a key member of our Morley practice, you will play a vital role in supporting our patients with chronic disease management.

Key Responsibilities
  • Develop and implement comprehensive care plans to help patients manage their chronic conditions
  • Empower patients to take ownership of their health and well-being
  • Provide coaching and guidance on health behavior change principles
  • Facilitate access to ongoing care from a multi-disciplinary team
  • Conduct health assessments to identify chronic disease early
  • Collaborate with GPs to promote preventative healthcare
  • Review and update patient care plans regularly
  • Manage administrative tasks related to CDM appointments
Requirements
  • AHPRA registered Enrolled or Registered Nurse
  • Proven experience in Chronic Disease Management and health assessments
  • Knowledge of Health Assessment is highly regarded
  • Strong organisational and time management skills
  • Outstanding communication and customer service skills
  • A passion for people and a personable approach
