**Key Responsibilities:**

• Provide mental health services to support the wellbeing of older adults.

• Conduct assessments, deliver interventions, provide counselling, and advocate for clients.

• Collaborate with a multidisciplinary team to develop and deliver person-centred care.

• Liaise with external service providers to ensure clients have access to appropriate community services.

• Support clients navigating the My Aged Care system, including CHSP, HCP, and STRC funding models.

• Work autonomously in the community while contributing to a supportive team environment.

**Requirements:**

• A Degree in Social Work from an accredited institution and membership with the Australian Association of Social Workers (AASW).

• Current Driver's License and a reliable vehicle (with KM reimbursement for work-related travel).

• Strong clinical skills in assessment, intervention, counselling, advocacy, and referral.

• Experience working with older adults, particularly those with chronic and complex health conditions.

• A solid understanding of the My Aged Care system and related funding programs.

• Excellent written and verbal communication skills to engage with clients, carers, colleagues, and external service providers.

• The ability to work independently while collaborating effectively with a multidisciplinary team.

• A commitment to person-centred care and a compassionate, client-focused approach.

• National Police Check (or willingness to obtain one).
