About this Position

This position is responsible for coordinating aged care, disability, mental health, and SEWB services across caseworkers, coordinators, and social workers.

  1. Coordinating services across multiple program streams.
  2. Leading and inspiring a diverse multidisciplinary team.
  3. Embedding quality frameworks into team operations.
  4. Promoting person-centred case management and compliance in care plans and assessments.
  5. Driving performance tracking and continuous improvement.
  6. Facilitating health promotion and community engagement initiatives.
  7. Ensuring safe, culturally respectful service delivery.
About You

To qualify, you will need tertiary qualification in Social Work, Psychology, or Mental Health, along with 2–3 years of demonstrated leadership experience in a relevant health or social services field.

  • Understanding of the Model of Care.
  • Strong interpersonal, written, and cross-cultural communication skills.
  • Ability to adapt, think critically, and drive strategic outcomes.
  • Accountability and financial awareness in service delivery settings.
