Care Coordinator for Elderly Services

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Infinite Care. As a key member of our staff, you will play a pivotal role in the wellbeing and engagement of our residents.
**Job Summary:**
- Coordinate and implement lifestyle programs that cater to the diverse needs and preferences of our residents.
**Responsibilities:**
- Assess resident engagement and enjoyment of lifestyle programs, making adjustments as needed.
- Work across personal care and lifestyle shifts, ensuring seamless delivery of care services.
- Collaborate with the Lifestyle Coordinator to develop engaging lifestyle programs.
- Organize activities that cater to residents' social, religious, and cultural requirements.
- Provide input during resident care assessments to ensure individualized and holistic programs.
- Assist with facility communications and coordinate celebrations for residents and staff.
- Develop and maintain strong relationships with residents, families, facility team members, volunteers, and local community groups.
- Manage and document risk, ensuring continuous improvements in our care services.
**Requirements:**
- Qualification in Aged Care: Cert 3 or 4 in Individual Support, Ageing, or Disability.
- 1 year's experience in a similar role, coordinating group activities and/or personal care.
- Innate empathy for residents and commitment to improving care quality and social environment.
- Ability to 'think on your feet', IT and tech savvy, excellent communication skills, and a confident team player.
- Valid NDIS Workers Screening Clearance (or ability to apply).
