Person-Centred Lifestyle Assistant Role

We are seeking a compassionate and skilled individual to join our team as a Person-Centred Lifestyle Assistant. This rewarding role involves delivering meaningful wellness programs that promote independence, health, and overall wellbeing for our residents.

  • Deliver person-centred wellness programs that enhance the daily lives of our residents.
  • Foster strong relationships with residents, families, and the care team to support holistic lifestyle outcomes.
  • Assess resident needs accurately and maintain thorough documentation to guide ongoing program improvement.
  • Work collaboratively with allied health professionals to ensure engaging and effective program delivery.
  • Contribute actively to quality improvement and a safe, positive workplace culture.
Requirements

To be successful in this role, you will have:

  • Certificate III in Aged Care or Leisure & Health, or working towards.
  • Strong communication and interpersonal skills with the ability to build rapport and advocate for residents.
  • Proven ability to deliver person-centred care and wellness programs.
  • Demonstrated teamwork skills and experience working collaboratively within a multidisciplinary environment.
