About the Role:
As a Care Manager at MiCare, you will play a pivotal role in enhancing the quality of life for elders living in the community. Your commitment to person-centred case management, client support, and astuteness in assessment, care planning, and review will be instrumental in ensuring that our clients receive the best possible care.
Responsibilities:
- Undertake initial assessments, care planning, ongoing reviews, and monitoring of elders in accordance with MiCare Home Care Policies and Commonwealth Government Aged Care guidelines.
- Liaise closely with internal and external stakeholders to ensure that elders receive services appropriate to their needs and in accordance with program guidelines.
- Initiate case and planning meetings as required, ensure solid documentation, and follow through on elder health, care, and service delivery.
- Develop individualized budgets for elders with Home Care Packages, ensuring spending compliance within the Home Care Package Guidelines.
- Organize referrals and permanent placements as needed.
- Provide support and advocacy for elders and carers as required.
- Be actively involved in quality reviews for the programs, ensuring your documentation and processes meet compliance requirements.
- Develop and maintain working relationships with relevant agencies and networks to promote MiCare.
- Respond to phone and online enquiries, as well as provide public education to potential clients.
About You:
- Demonstrated knowledge of sound case management within the aged care sector, preferably working with complex family and social issues.
- Clear understanding of the Aged Care Quality Standards.
- Solid understanding of Commonwealth Government community aged care programs (Commonwealth Home Support Program, Home Care Package program, and Veterans Home Care).
- Proactive, reliable, with excellent communication skills and attention to detail.
- Ability to work with others and be a participative and supportive team member.
- Ability to meet compliance requirements of the role, including current First Aid and CPR certification, valid Victorian driver's license with an insured and registered vehicle, and ability to provide evidence of current flu and COVID-19 vaccination.
Qualifications:
- Tertiary qualification in Allied Health, Nursing, or Social Work.
