**Job Description:**
We're looking for an experienced Admissions Nurse to join our team as a Healthcare Transition Manager. In this role, you'll play a critical part in ensuring the smooth transition of residents into our care facilities. You'll work closely with our clinical team, admissions coordinator, and other stakeholders to deliver exceptional customer service and ensure that our residents receive the highest level of care.
**Key Responsibilities:**
- Develop and implement individualized care plans to meet resident needs.
- Collaborate with clinical team to assess resident needs and develop effective communication strategies.
- Liaise with social workers, GPs, and hospital staff to clarify referral information and facilitate effective communication.
- Conduct regular quality assurance audits to ensure high-quality care is being delivered.
**Requirements:**
- AHPRA registered as an RN or EN.
- Extensive nursing experience in aged care or similar.
- Demonstrated experience working collaboratively in a clinical environment with numerous stakeholders.
- Superior communication skills and ability to confidently use clinical information systems.
