Occupational Therapist - Community Care Role

This is an exciting opportunity to provide evidence-based clinical care in the community, focusing on home care packages, Commonwealth Home Support Program, and hospital substitution programs.

As a Community Occupational Therapist, you will deliver targeted occupational therapy for clients with complex health conditions or age-related care needs. Your responsibilities will include prescribing aids, equipment, and home modifications to support client goals, collaborating with a multidisciplinary team for optimal outcomes, and participating in our clinical supervision program.

To be successful in this role, you will need:

  • Community or hospital OT experience preferred;
