We are seeking a highly skilled Care Coordination Specialist to join our team.

As a Care Coordination Specialist, you will play a critical role in ensuring the delivery of high-quality care services to our clients. Your primary responsibility will be to develop and implement individualized care plans that meet the unique needs and goals of each client.

The ideal candidate will have a strong background in nursing and previous experience in case management. You will be required to work collaboratively with internal and external stakeholders to coordinate the delivery of care services, including managing budgets and advocating for clients.

This is a full-time position offered on a fixed-term contract, with the possibility of extension based on funding. Our comprehensive training and orientation program will support you in your new role.

Key Responsibilities:

  • Develop and implement individualized care plans that meet the unique needs and goals of each client
  • Coordinate the delivery of care services, including managing budgets and advocating for clients
  • Work collaboratively with internal and external stakeholders to ensure seamless delivery of care services
  • Participate in case conferencing and scheduled clinical appointments as required

Requirements:

  • Tertiary qualifications in Nursing (with relevant registration)
  • Previous Case Management Experience (Community and Aged Care experience highly desired)
  • Demonstrated ability to develop and maintain strong networks with internal and external stakeholders
  • Current drivers' license and comfortable traveling throughout the region each day
  • COVID-19 Vaccination
