We are seeking a Support Care Coordinator to join our team in East Kew. As a key member of our disability support team, you will provide person-centered care to support individuals with complex disabilities.

As a Support Care Coordinator, your role will involve providing daily living assistance, including personal care and medication management. You will work collaboratively with our clients and their families to develop personalized plans that meet their unique needs and goals.

Key Responsibilities
  • Provide daily living assistance, including personal care and medication management
  • Develop and implement personalized care plans in collaboration with clients and their families
  • Assist clients with various aspects of daily life, including shopping, cooking, and household tasks
  • Support clients to achieve their goals and maintain independence
Requirements
  • Diploma or Certificate IV in Community Services or related field (or equivalent experience)
  • Valid NDIS check, CPR, and First Aid Certificate
  • Fully COVID vaccinated
  • Right to work in Australia
