**Job Description**

We are seeking a compassionate and experienced Care Manager to join our team. As a key leader in our residential aged care community, you will play a vital role in ensuring the delivery of safe, high-quality, resident-centred care.

The ideal candidate will have strong leadership and team management skills, excellent knowledge of the Aged Care Quality Standards, and a genuine commitment to supporting residents to live their way with dignity and grace.

**Key Responsibilities:**

  • Lead and support the care team to deliver holistic, person-centred care.
  • Oversee care planning, clinical governance, and resident assessments.
  • Ensure compliance with regulatory requirements, including the Aged Care Act 1997.
  • Promote resident choice, family engagement, and a strong sense of home.
  • Drive continuous improvement in quality, safety, and resident experience.

**Required Skills and Qualifications:**

  • Current registration with AHPRA as a Registered Nurse.
  • Minimum 5 years' experience as a Clinical Nurse, Care Coordinator, or Care Manager (with at least 1 year in aged care).
