This is an opportunity to work with a dedicated, multidisciplinary team to support clients overcome barriers to accessing support, connect with community resources, and build independence in daily life.
The Universal Aftercare Service is designed to reach and support communities who may not typically engage with the health system. We are keen to talk with people who have an interest in working within a recovery framework, collaborating with other relevant services, and supporting individuals to build independence and contribute to their recovery, community integration, and wellbeing.
