Job Title: Support Worker

We are seeking an experienced and skilled professional to join our Assertive Case Management (ACM) program. The ACM program provides intensive care coordination to support individuals with chronic and severe substance use issues.

The successful candidate will work as part of a multi-disciplinary team to provide support, stabilise and re-integrate clients into their communities, and assist in managing underlying cognitive impairment.

Key Responsibilities:

  • Provide case management, crisis management, advocacy, and education to individuals with complex needs
  • Develop person-centred, holistic care plans that promote harm reduction and address drug and alcohol issues
  • Collaborate with other care providers to deliver effective interventions

Requirements:

  • Clinical expertise in managing alcohol and other drug issues
  • Experience in discipline-specific assessment and interventions
  • Knowledge of the harm reduction approach and its application in practice
  • High-level interpersonal and written communication skills to engage with clients and stakeholders
  • A current NSW Driver's Licence is essential for this role
