Clinical Care Specialist

We are seeking a skilled Registered Nurse to join our team as a Clinical Care Specialist. As a key member of our At Home Support team, you will work independently with support from experienced senior clinicians to provide high-quality clinical and care services to our clients.
About the RoleIn this role, you will be responsible for implementing and monitoring care plans to enable clients to live well at home. You will develop therapeutic and caring relationships with clients, collaborating with health professionals to achieve positive outcomes.
As a Clinical Care Specialist, you will have the opportunity to work in a variety of practice areas, assisting clients to experience a greater level of independence and ensuring their health and wellness are supported.
