Job Summary

Mecwacare is seeking a highly skilled Disability Liaison Officer to join our team. As a Disability Liaison Officer, you will play a critical role in ensuring customer-centric work practices are in place, facilitating access to NDIS services, and developing care plans with participants.

Key Responsibilities

  • Facilitate access to NDIS services and develop care plans with participants
  • Complete assessment paperwork for service access and annual review
  • Build networks and promote community engagement
  • Provide administrative support and facilitate effective customer engagement

Requirements

  • Certificate IV in Disability or equivalent
  • First Aid level 2 Certificate and NDIS Check
  • Rostering and scheduling experience in aged and disability service environment
  • Proactive attitude and ability to multitask
  • Demonstrated experience in a client-focused position with a commitment to providing superior customer service
  • Logical mind with an ability to see the bigger picture
  • Strong administrative and organisational skills
  • Strong IT skills including Microsoft Office suite
  • Knowledge of Carelink+, CHSP, HACC guidelines and My Aged Care Portal
