Community Services Specialist

We are seeking a Community Services Specialist to join our team, providing high quality, person centred services to individuals and families in need.

The ideal candidate will have experience in client assessment and case management, with a tertiary qualification in Community Services or a related field. A degree level qualification is preferred.

The role involves providing over-the-phone intervention, triage and case management, as well as establishing relationships with clients that encourage self-determination and participation.

In this role, you will work collaboratively with the First Response team to provide a comprehensive response to client needs, using frameworks and guidelines to inform decision-making.

You will also be required to work in a mobile support capacity, meeting clients in community, and maintaining professional and productive relationships with stakeholders.

This is a permanent, full-time position located in two positions, Spring Hill and Southport, QLD. Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services CRISIS Level 2.1 Award.

Key Responsibilities
  • Answer calls in a timely manner, providing initial response and assessment, inclusive of needs, risks and vulnerabilities
  • Responding to Walk ins on site and providing assessment and referral as appropriate
  • Establishing relationships with individuals who access the service, that encourage self-determination and participation
  • Complete brief intervention tasks such as providing information and referrals to internal and external stakeholders
  • Liaise daily with First Response team members to provide collaborative and comprehensive response to client need
  • Will be required to work in a mobile support capacity to meet participants in community (expected to do this at least 4 out of 5 days per week)
  • Licensed and able to operate work vehicle
  • Maintain professional and productive relationships with stakeholders
  • Ability to work autonomously (making decisions with minimal guidance, utilising frameworks and guidelines to make decisions, time/workload management, etc.)
Requirements
  • QLD Working with Children Check
  • Driver's Licence
  • First Aid Certificate
  • Tertiary qualification in Community Services, Social Work or related field. A degree level qualification is preferred.
  • Relevant experience in a social service environment including in client assessment and case management.
  • Previous experience providing case management type support over the phone preferred.
