Job Summary

We are seeking a skilled healthcare professional to join our team as a Community Care Nurse. This is an exciting opportunity for a registered nurse to make a real difference in the lives of our clients.

The successful candidate will provide high-quality nursing care, undertake assessments, and develop personalized care plans to support the needs of our clients.

  • Nursing Care Specialist: Provide compassionate and professional nursing care to clients in their homes.
  • Client Advocate: Work collaboratively with clients, their families, and other healthcare professionals to achieve optimal outcomes.
  • Community Leader: Contribute to policy development and service delivery through expert advice and guidance.

About the Role

This role is responsible for providing in-home nursing care, undertaking assessments, and developing care plans for clients. The successful candidate will work independently and as part of a team to deliver high-quality services.

Key Responsibilities:

  • Provide comprehensive nursing care to clients in their homes, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans that meet the unique needs of each client.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimize client outcomes.
  • Contribute to the development and implementation of policies and procedures that enhance the quality of our services.
  • Provide leadership and support to enrolled nurses and other healthcare professionals within the region.

What We're Looking For

To be successful in this role, you will need:

  • A Bachelor of Nursing Degree or equivalent qualification.
  • Minimum 5 years of experience in community nursing or a related field.
  • A current C class Driver's License (QLD).

  • Excellent communication and interpersonal skills.
  • Ability to assess client needs and develop appropriate plans for interventions and treatment.
  • Strong problem-solving and critical thinking skills.
