**Job Title:** Connections Transitions Coordinator

We are seeking an experienced professional to join our team as a Connections Transitions Coordinator. In this role, you will be responsible for ensuring that patients receive seamless continuity of care upon their release from correctional centres.

The Connections Program is designed to improve the lives of individuals struggling with drug and alcohol addiction. As a key member of our team, you will work closely with community-based networks to provide tailored support and services to meet the unique needs of each patient.

To succeed in this role, you must have a degree in a relevant field or equivalent experience. You should also possess excellent communication skills, both written and verbal, as well as strong networking and interpersonal abilities. Additionally, you should be able to work independently and as part of a multi-disciplinary team, meeting tight deadlines and resolving conflicts effectively.

A comprehensive assessment and post-release care plan will be developed for each patient, taking into account their individual needs and circumstances. You will be responsible for liaising with health and welfare service providers to ensure a smooth transition of care, and for following up on referrals received to address any issues that may arise.
