**Job Title:** Connection Coordinator

We are seeking an experienced individual to join our team as a Connection Coordinator. In this role, you will be responsible for coordinating the transition of patients from correctional centres to community-based services.

**Key Responsibilities:

  1. Assess patient needs and develop individualized care plans
  2. Coordinate with community-based service providers to ensure seamless transitions
  3. Maintain accurate records and reports

**Requirements:

  • Relevant qualifications in health or social sciences
  • Experience working with vulnerable populations
