Aged Care Specialist

Key Responsibilities- Conduct annual assessments and care plan reviews
- Perform a wide range of clinical assessments (Falls Risk, Cognitive, Continence, Pain, Skin Integrity, Wound care, Medication Management Capacity, Carer Strain Index)
- Develop treatment plans and make timely referrals
- Recommend suitable aids and equipment
- Provide wound care and assist with continence management and catheter care
- Liaise with the Client Care Manager (CCM) around the care and supports provided to our home care clients
You're a caring and capable RN who is confident working independently in the community. You bring clinical know-how, a person-first approach, and a genuine interest in helping older Australians live their best life.
Essential Requirements- Current AHPRA registration with at least 2 years of nursing experience
- Experience in aged care or community home care (preferred, but not essential)
- First Aid (HLTAID011 or equivalent) and CPR (HLTAID009), or willing to obtain within one month
- Strong communication and relationship-building skills
- Reliable vehicle and willingness to travel between client homes (travel reimbursed per kilometre)
- A warm, empathetic approach and a commitment to delivering excellent care
