Job Summary:

We are seeking a qualified Clinical Assessor to join our team in delivering the Aged Care Assessment Program.

Key Responsibilities:
  • Conduct comprehensive assessments across physical, medical, psychological, cultural, and social care dimensions.
  • Offer consumers appropriate service options, including residential care, respite, community packaged care, and other relevant programs.
  • Collaborate with clients to assess their needs, goals, and preferences, focusing on wellness and reablement to promote independence.
