About the Role:
">We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Care Coordinator. As a key member of our care team, you will play a vital role in ensuring the highest level of clinical care for our residents.
">Duties:

- Liaise with care staff, medical professionals, allied health practitioners, hospitals, and specialist health practitioners to ensure our residents receive high-quality clinical care.">
- Complete individualized assessments for all new residents, including respite residents, ensuring they meet current guidelines in conjunction with authorized representatives.">
- Review individualized care plans for residents and their representatives, updating these according to written guidelines/checklists.">
- Undertake clinical care for residents, including complex wound care, pain management, behavior management, end-of-life care, and other clinical procedures.">
- Ensure medication management, including pharmacy liaison, drug checking, dangerous drugs storage, and recording, medication administration, and reporting any adverse drug reactions.">
Qualifications and Experience:

- Current registration with the Nurses Board of Australia via the Australian Health Practitioners Regulation Agency as a Registered Nurse.">
- Bachelor of Nursing (or equivalent).">
