Home Care Case Manager Role

We are seeking a dedicated professional to fill the role of Home Care Case Manager, where you will be responsible for providing support and guidance to older individuals in their homes. This position is ideal for those who have experience in case management, aged care, or a related field.

Key Responsibilities:
  • Conducting comprehensive assessments and developing individualised care plans
  • Meeting with new clients and supporting them through the onboarding process
  • Working collaboratively with families, health professionals, and service providers to deliver high-quality care
  • Monitoring client needs and reviewing progress to ensure timely adjustments are made
  • Providing safe, person-centred, and enablement-focused care that maximises funding and promotes the best possible health and wellbeing outcomes

This is a varied and rewarding position that requires a balance of professional expertise, compassion, advocacy, and problem-solving skills. You will be expected to work independently and as part of a team to ensure our clients feel respected, valued, and supported.

What We're Looking For

We are seeking an individual who brings:

  • Experience in case management, aged care, home care, or a related field (typically 3+ years)
  • Knowledge of the aged care sector, including quality standards and codes of conduct
  • Strong communication and relationship-building skills
  • A client-focused approach with confidence in advocacy and decision-making
  • Experience in budgeting and fund management
  • Solid IT and documentation skills

PREFERRED QUALIFICATIONS: Certificate IV in Home & Community Care / Aged Care, or a relevant bachelor's degree (e.g., Nursing).
