Job Overview

The successful candidate will work alongside Allied Health and nursing teams to implement and maintain clients' personalized care and support needs. The ideal candidate will be compassionate, knowledgeable, and caring, with a strong skillset and willingness to learn.

Key Responsibilities

  • Implement and maintain clients' care plans
  • Provide support and assistance to clients as needed
  • Collaborate with Allied Health and nursing teams to ensure comprehensive care
  • Conduct regular assessments and evaluations of client progress
  • Document all interactions and updates in client files

Requirements

To succeed in this role, you will need:

  • A current WA licence
  • A reliable vehicle with comprehensive insurance
  • An NDIS screening check (or willingness to obtain)
  • A current First Aid Certificate
  • A current CPR Certificate
  • A current Manual Handling Certificate
  • A current Medication Certificate
