Virtual Care Service Specialist

We are seeking a skilled Remote Healthcare Service Professional to join our team.
This role involves delivering healthcare as a member of a multidisciplinary team within a virtual care framework, ensuring optimal health outcomes for consumers, carers and families.
About the Role:This is an exciting opportunity for experienced social workers who want to leverage their skills in providing high-level services remotely. You will work closely with a multidisciplinary team to deliver patient-centered care, promoting healthy outcomes and quality of life for individuals and families.
Key Responsibilities:- Provide high-level remote social work services to consumers, carers and families
- Ensure the provision of optimal health outcomes consistent with policies, procedures, standards and relevant legislation
- Bachelor or postgraduate degree in Social Work
- Eligibility for Australian Association of Social Workers (AASW)
- Extensive clinical experience, including experience as a Level 2 Social Work
- Proficient written, verbal and interpersonal communication skills
- Demonstrated computer literacy and ability to use health information systems
- Good digital literacy and proficient skills in current social work practice within a Virtual Care framework
