Aged Care Clinical Lead

About this role:
- This position offers a unique opportunity to lead and manage clinical services in a residential aged care facility.
- You will oversee the delivery of high-quality, person-centered care to residents and collaborate with the multidisciplinary team to achieve excellent outcomes.
Key Responsibilities:
- Conduct comprehensive clinical assessments and reviews of resident care needs, ensuring individualized plans are developed and implemented effectively.
- Facilitate and coordinate the admission process for new residents, ensuring seamless transitions into our care community.
- Lead by example, promoting a collaborative approach among clinical and care staff to deliver exceptional, person-centered care that exceeds resident expectations.
- Support the development and clinical capability of the clinical care team, fostering growth and innovation within the team.
- Monitor and minimize clinical risk, adhering to quality standards and regulatory requirements.
- Identify changes in resident care needs, supporting accurate classification and funding through AN-ACC processes.
- Review and investigate clinical incidents, implementing corrective actions to prevent recurrence.
Requirements:
- Current registration as a Registered Nurse with AHPRA.
- Relevant postgraduate studies desirable.
- Demonstrated experience in leadership roles within aged care, showcasing effective communication, problem-solving skills, and teamwork abilities.
- A comprehensive understanding of Legislation governing aged care, including best practice guidelines and professional codes of conduct.
- Working knowledge of continuous improvement principles, the Aged Care Act, Aged Care Quality Standards, AN-ACC funding, assessment processes, and care planning.
- Familiarity with OSH requirements in healthcare settings.
- Satisfactory completion of Key Personnel checks, including Bankruptcy and Police Clearance.
