
Care Coordinator – Family Support

**Job Overview**
Kanda is seeking a skilled and dedicated professional to provide intensive family support services.
As a Family Support Practitioner, you will work closely with families in their homes and communities, providing tailored support to children, young people, and their families.
Key Responsibilities Include:
- Providing intensive family support services, including parenting strategies and practical life skills coaching.
- Working with families to access community resources, education, and employment pathways.
- Conducting home visits and engaging in early intervention strategies to improve safety and wellbeing outcomes.
- Collaborating with Child Safety and other government and community stakeholders to deliver wrap-around support.
**Required Skills and Qualifications**
To be successful in this role, you will need:
- A relevant Bachelor's Degree, Diploma or Certificate IV in Community Services, Youth/Social Work, or a related field.
- Experience working with young people/children, preferably in a family support or related field.
- A valid Drivers Licence and access to a vehicle.
- The ability to work across a full shift pattern (mornings, evenings and sleepovers).
- The willingness to obtain a Working with Children Blue Card and NDIS Worker Screening Check prior to commencement.
