">We are seeking a skilled and dedicated Community Nurse Liaison to join our Complex Planning and Pathways team.
">The successful applicant will play a vital role in identifying and facilitating patients who require discharge from bed-based services to the Community Nursing program with appropriate supports in place.

">About the Role:
">The CN liaison will be responsible for monitoring current Community Nursing patients in bed-based services and ensuring adequate and timely discharge planning for transition to the community setting.
">The ideal candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
">Key Responsibilities:
">Monitor current Community Nursing patients in bed-based services and ensure adequate and timely discharge planning for transition to the community setting.
">Identify and facilitate patients who require discharge from bed-based services to the Community Nursing program with appropriate supports in place.
">Work collaboratively with the Complex Planning and Pathways team to develop and implement individualized care plans.
">Required Skills and Qualifications:
">A relevant tertiary qualification in nursing or a related field.
">Current registration as a nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
">Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
