About the Role

We are seeking a dedicated Case Manager to join our Community Health and Inclusion team. As a Case Manager, you will play a vital role in providing intensive case management to clients referred from the Exceptional Needs Unit.

Your primary responsibility will be to support clients experiencing long-term and/or multi-layered disadvantage in various areas, including physical and mental health, economic capacity, and more. You will work under the direction of the Manager, Community Connections Program, and report directly to them.

The successful candidate will have a Bachelor of Social Work (AASW-recognised) or equivalent, along with experience in case management-focused service delivery and general principles of risk assessment and case management. They will also possess excellent communication and interpersonal skills, allowing them to effectively engage with clients and build strong relationships.

Key Responsibilities:

  • Complete professional intake and assessment of clients.
  • Create and manage actionable care plans with clients, integrating information provided at referral and gathered through client assessment.
  • Work flexibly with clients, engaging them in adaptive ways and utilizing various tools as appropriate, while developing their capacity to engage more broadly.
  • Connect clients with purposeful partners.

The ideal candidate will have the following skills and experience:

  • Bachelor of Social Work (AASW-recognised) or equivalent.
  • Experience in case management-focused service delivery.
  • Experience with the general principles of risk assessment, and case management more broadly.

Requirements

To be considered for this role, you must meet the following requirements:

  • A current and valid DHS: Child-Related Employment Screening/ WWCC.
  • A current and valid National Police Clearance.
  • Training - Child Safe Environments.
  • A valid full South Australian drivers' license (C Class).
  • Ability and willingness to travel within South Australia as required.
