Youth Disability Case Manager">

As a key professional in our team, you will play a vital role in making a difference in the lives of young people with disability. We are seeking an experienced Youth Disability Case Manager to join our dynamic team and contribute to creating brighter futures for these individuals.

This is an exciting opportunity for a skilled practitioner who is passionate about providing trauma-informed support and empowering young people to achieve their goals.

In this challenging and rewarding role, you will work collaboratively with internal teams and external stakeholders to develop and implement Individualised Support Plans, coordinate services, and foster environments that promote emotional, social, physical, and cultural wellbeing.

We are looking for a seasoned professional who has experience in case management within human services and a strong understanding of trauma-informed practice and disability support frameworks.

The successful candidate will possess excellent communication, leadership, and stakeholder engagement skills, as well as the ability to work independently and manage a full workload.

This role requires a valid QLD Driver's Licence and eligibility for Blue Card, Yellow Card, and relevant screenings.

    • You have a minimum 2 years' experience in case management within human services">
    • You have knowledge of trauma-informed practice and disability support frameworks">
    • You have experience working with young people in out-of-home care or disability contexts">
    • You have strong communication, leadership, and stakeholder engagement skills">
    • You are able to work independently and manage a full workload">
    • You understand NDIS, safeguarding frameworks, and child safety legislation">
    • You hold a valid QLD Driver's Licence and are eligible for Blue Card, Yellow Card, and relevant screenings">
