About the Role:

Sunbury and Cobaw Community Health is seeking a skilled Occupational Therapist to join our dynamic team. As an Occupational Therapist with our organization, you will play a vital role in delivering high-quality services to our community.

Key Responsibilities:

  • Conduct goal-directed care planning, case coordination, equipment prescription, and home modification to support individuals with disabilities and aged care needs
  • Develop and implement effective therapy plans to promote independence, mobility, and overall well-being
  • Collaborate with multidisciplinary teams to provide comprehensive care and support services
  • Communicate effectively with clients, families, carers, and other service providers to ensure seamless care delivery
  • Participate in program development, innovation, and quality improvement initiatives to enhance service delivery and outcomes

Requirements:

  • A minimum of two years of clinical occupational therapy experience
  • Excellent interpersonal skills and ability to communicate effectively with diverse stakeholders
  • Knowledge and experience working with My Aged Care, State Wide Equipment Program, and NDIS
  • Ability to work as part of a multidisciplinary team and contribute to collaborative practice

Selection Criteria:

  • Demonstrated experience and skills in occupational therapy practice, particularly in community health settings
  • Strong communication and interpersonal skills, with ability to work collaboratively with diverse stakeholders
  • Knowledge and experience working with relevant legislation, policies, and guidelines
  • Ability to work autonomously and as part of a team, with strong problem-solving and analytical skills
