About the Role:
Care Connect is seeking an experienced and driven professional to fill a part-time (30.4 hours per week), fixed-term position as a Care Manager. This role involves providing specialist and intensive case management to individuals in the community who require assistance to navigate and apply for aged care funded packages and to access other supports within the community.
Key Responsibilities:
- Engage with key stakeholders and local intermediaries to build and maintain a network of support services.
- Collaborate and communicate with clients, families, and local intermediaries to achieve positive outcomes.
- Provide assertive outreach to ensure that Care Connect services are accessible for those who need them.
- Assess clients' needs and make appropriate referrals/linkages with other services in the community to support their personal wellbeing.
- Provide information, support, and resources to develop clients' knowledge, life skills, and capacity for independence, including self-advocacy.
Requirements:
- Diploma or Degree in a Health or Allied Health field, or minimum of 4 years relevant experience to equate to Diploma or Degree level in Health or Allied Health.
- Strong communication, advocacy, and interpersonal skills.
- Demonstrated time management, prioritization, and budget management skills.
- Experience in the Microsoft Office suite and exposure to and/or ability to acquire skills for in-house client database systems.
- A current Australian Driver's Licence and access to a reliable vehicle.
- Immunization record in line with VIC Health vaccination policy, or willingness to obtain, including Covid-19 vaccinations.
