As a Disability Service Coordinator, you will play a vital role in empowering individuals with disabilities through innovative solutions and support.

This is a full-time on-site position that requires strong organizational and communication skills, as well as the ability to work collaboratively with families and support networks.

Key responsibilities include:

  • Developing personalized support plans to meet the unique needs of each individual
  • Coordinating with various support teams and external agencies to ensure seamless service delivery
  • Managing operations tasks to optimize efficiency and effectiveness
  • Providing exceptional customer service to clients and their families
  • Facilitating effective communication between all parties involved

Qualifications include:

  • Service Coordination skills
  • Strong Interpersonal Skills and Communication abilities
  • Operations Management experience
  • Excellent Customer Service skills
  • Ability to work collaboratively with families and support networks
  • Empathy and sensitivity in handling cases with mental health histories and forensic challenges
  • Bachelor's degree in Social Work, Psychology, or a related field is desirable, but not essential
  • Experience in the disability services sector is essential
