Care Coordinator Position

We are seeking a highly skilled and compassionate individual to join our team as a Care Coordinator. This role will play a vital part in planning and managing care services for clients receiving in-home support.

Key Responsibilities:
  • Conduct initial assessments and develop person-centred care plans.
  • Coordinate and schedule care services with accuracy and efficiency.
  • Support carers with guidance, resources, and regular check-ins.
  • Maintain compliance with care standards and documentation.
  • Ensure clients feel heard, respected, and supported at all times.
Requirements:
  • Experience in care coordination, case management, or a similar role.
  • A background in aged care, disability support, or community services.
  • Excellent communication, time management, and problem-solving skills.
  • A strong ethical approach and attention to detail.
  • Confidence using technology and care management systems.
  • A current driver's licence and access to a reliable vehicle.

Qualifications:

Certificate IV or Diploma in Aged Care, Disability, Community Services or similar (preferred) with minimum 2 years' experience.

Knowledge of Home Care Packages (HCP) and or NDIS.
