About the Role
The Care Manager - Home Care is a key position within our Home Care portfolio, responsible for leading the achievement of high-quality clinical and care services in the Southern region. This role will be based at our Central Office, coordinating resources and activities to deliver person-centred care for clients that is responsive to their assessed needs.
Key Responsibilities
- Ensure all clients in the region have a current and holistic assessment and a service plan that manages all risks associated with their assessed needs, within their service budget and with a health-promoting focus.
- Actively engage with the community within the region, promoting Southern Cross Care as a Provider of Choice for Support at Home services to older people.
- Act as an advocate for clients to assist with making informed decisions.
- Monitor the quality of comprehensive care planning strategies to ensure clients' quality of life and independence is optimised, while managing risks and duty of care.
- Ensure all assessments and documentation meet legislative requirements.
About You
We are seeking a highly skilled and experienced Care Manager to join our team. The successful candidate will have a Bachelor of Nursing or recognised equivalent, current registration and practising certificate with AHPRA as a Registered Nurse, and strong organisational and time management skills.
