Key Responsibilities:

  • Conduct comprehensive assessments and develop tailored care plans to cater to the diverse needs of our residents.
  • Provide expert advice on service options and coordinate referrals to ensure seamless transitions for our clients.
  • Empower clients and carers to manage their care plans with confidence by fostering open communication and support networks.
  • Maintain accurate and up-to-date client records, ensuring compliance with relevant funding and legislative requirements.
  • Monitor and evaluate service delivery, responding promptly to changing client needs and preferences.

About You:

  • A tertiary qualification in Health or a related field, such as nursing or allied health.
  • Significant experience in case management and care planning, preferably in aged care.
  • A strong understanding of aged care reforms and consumer-directed care principles.
  • Excellent communication, time management, and interpersonal skills, with the ability to build strong relationships with clients, carers, and other stakeholders.
  • A current Victorian driver's licence is highly desirable.
