We are seeking an experienced and passionate support worker to join our community in Melbourne.
Key Responsibilities- Support individuals with disabilities in a person-centred approach, focusing on their unique needs and abilities.
- Develop and maintain relationships with customers, their families, and carers.
- Work collaboratively with the team to create a supportive and inclusive environment.
- Contribute to the development and implementation of individualised plans to achieve customer outcomes.
- Monitor and report on customer progress and adjust support plans as needed.
- Qualifications: Advanced Diploma in Disability or equivalent qualification.
- NDIS Worker Screening Check Clearance and Working with Children's Check (Employee).
- Current Level 2 First Aid & CPR Certificate.
- Full Victorian Driver's License.
- Valid Australian Working Rights and an International Police Check if applicable.
