We are seeking a full-time Health Linker to join our team and elevate our mission of providing comprehensive support to clients engaged in the WARATAH Program.

Key Responsibilities:
  • Provide coordinated support to clients, crafting personalised plans that enhance health and well-being, re-establishing connections to community groups and essential services.
  • Help individuals tackle broader challenges affecting their health, such as financial difficulties, housing issues, unemployment, isolation, and caregiving responsibilities.
  • Offer tailored guidance to clients, their families, and caregivers to boost health literacy and promote healthy lifestyle choices.
Requirements:
  • Relevant tertiary qualifications in Nursing, Aboriginal Health, Allied Health, Community/Human Services, Social Work/Social Science, or equivalent.
  • Solid understanding of the broader determinants of health—including social, economic, and environmental factors—and their impact on communities.
  • Proven experience in co-developing person-centred support plans that prioritise client well-being and aspirations.
  • Advocacy skills to enhance client access to programs and inspire community participation.
