The Role

As a Support Worker at our organisation, you will be responsible for co-ordinating personal care for residents in accordance with care plans that meet their individual needs and preferences.

You will work within a multidisciplinary team to provide effective clinical and emotional support to residents, recognising changes in their conditions and escalating observations and concerns.

Your Responsibilities

  • Deliver high-quality, person-centered care to residents and families, including consideration for cultural, spiritual and individual needs.
  • Provide personal care to residents with complex needs, such as palliative care, dementia support, restorative care and pain management, to enhance their wellbeing.
  • Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes.
  • Monitor the cleanliness, safety, environment and promptly report issues of concern.

About You

To be successful in this role, you will need:

  • Certificate III in Individual Support, or equivalent (Aged Care).
  • Experience in residential aged care/home care.
  • Experience with dementia care and palliative care support.
  • Demonstrated ability to support residents in a fast-paced environment with exemplary communication skills in a team and one-on-one interactions with all levels of care partners.
  • Commitment to being part of a workplace culture based on strong relationships and teamwork.

Essentials for Working in Aged Care

To work in aged care, you will need:

  • A current Working With Children Check card.
  • Current and ongoing flu and COVID-19 Vaccinations.
  • Australian working rights.
  • Ability to pass a national police clearance and NDIS Clearance.
