About the Role

  • Building strong relationships with clients and their families, providing personalized support and care.
  • Conducting comprehensive client assessments and developing tailored care plans to meet the unique needs of each individual.
  • Providing clinical guidance and advice to home care workers, ensuring they have the necessary skills and knowledge to deliver excellent care.
  • Liaising with clients and families on a regular basis, addressing any concerns or issues that may arise and ensuring they are satisfied with the level of care provided.
  • Maintaining accurate records and updating client information to ensure seamless care coordination.
Requirements

To be successful in this role, you must possess:

  • A formal tertiary qualification in Nursing, such as a Bachelor of Nursing.
  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Demonstrated experience in the aged care sector.
  • A National Criminal History Check and Working with Children Check.
  • A valid First Aid and CPR certificate.
