Transition Care Nurse Role

Join our healthcare team as a Transition Care Nurse and make a real difference in patient outcomes. This innovative model of care utilises cutting-edge technology to support patients who no longer require acute care clinical services.

Your opportunity will involve working in an alternate environment that focuses on restoring health, with the option to work full-time or part-time on a 7-day rotating roster across morning, afternoon, and night shifts.

  • Monitor patients' progress and report changes to a broader multidisciplinary team
  • Contribute to care planning and patient assessments
  • Support activities of daily living and administer medications
About You

To succeed in this role, you will need:

  • At least one year's experience as an Endorsed Enrolled Nurse
  • Experience in wound care management
  • Current First Aid and CPR certificates
  • A high attention to detail with a patient-focused approach
  • A South Australian Working with Children Check and National Police Clearance (or willingness and eligibility to obtain)
  • Ability to provide proof of up-to-date vaccinations
