Job Description

We're seeking a highly skilled and compassionate Care Coordination Specialist to join our team. As a key member of our support services, you'll play a vital role in empowering individuals with disabilities to live independently and dignified lives.

Your primary responsibility will be to support participants in understanding and implementing their NDIS plans. You'll work closely with them to coordinate services such as allied health, personal care, and social supports, ensuring they receive the best possible outcomes.

You'll be part of a dynamic team that values collaboration, empathy, and open communication. We believe in fostering strong relationships with our clients and delivering person-centred support that genuinely makes a difference in their lives.

Main Responsibilities
  • Supporting participants to understand and implement their NDIS plans
  • Coordinating services such as allied health, personal care, and social supports
  • Building participant capacity for independence and choice
  • Managing a caseload with professionalism and compassion
  • Keeeping accurate and timely case notes and reports
Requirements
  • Experience in NDIS Support Coordination or a similar role (preferred)
  • Solid knowledge of the NDIS, funding categories, and provider networks
  • A passion for person-centred support and inclusive practice
  • Strong time management and problem-solving skills
  • Certificate IV or Diploma in Community Services, Disability, or related field (desirable)
  • Valid driver's licence and NDIS Worker Screening Check
