Role Responsibilities

As a Client Care Coordinator, your responsibilities will include:
- Establishing strong relationships with clients and their families
- Evaluating client needs and available resources for health, welfare, and other services
- Conducting comprehensive Client Care Assessments and Care Planning
- Maintaining precise records in accordance with Aged Care Quality Standards and NDIS Practice Standards
- Assisting clients in achieving their personal goals
- Providing support and feedback to Home Care Workers
- Adjusting work priorities to meet the demands of a dynamic environment
- Upholding the company’s values and planning support using a modern approach
Essential Qualifications:
- Experience in assessing and planning care needs
- A minimum of 3 years of experience as a Support Worker
- Strong understanding of Aged Care Quality Standards and NDIS Practice Standards
- Excellent organizational, administrative, and time management skills
- Eligibility to work in Australia
- COVID-19 vaccinations
Required Certifications:
- National Police Check
- Blue Card
- Driver's License
- NDIS Worker Screening Check
- First Aid & CPR Certification
Job Type: Full-time
Salary: Starting from $75,000.00 per year
Work Schedule:
- Monday to Friday
- On-call availability
